To download the submission form please login. You can use Adobe Acrobat Reader to electronically fill patient details and test requested.

Results are available via our lab webpage immediately, and a full report is emailed each day. A follow up fax or hard copy by US mail will follow later. So please be sure to enter an e-mail address on your submission forms to ensure the earliest possible reporting of results.

The accession numbers of recent samples from your clinic are displayed to the left, clicking on the accession number will open a window with the results from that accession, including reference ranges and interpretations.

Once you are logged on, you can obtain an submission form customized for your clinic, with address and contact details already filled out from our records.

• Your e-mail address is your username.
• The clinic ID number is your password.
  • On our result reports, your clinic ID number is given in the upper left corner of the page, directly below the email address we have recorded for your clinic.
• If the user ID shows “Call to Initialize”, we do not have an email address recorded for your clinic.
  • Please email (gilab@cvm.tamu.edu) us with an email address for your clinic to start using this system.
